The St. Charles Borromeo Athletic Association appreciates parent and student support, through player fees, in helping to offset the annual costs of sponsoring athletic programs for students. The student-athlete fee is $25 per sport played. The fees cover the cost of CYO fees, uniforms, referees, grounds' upkeep, equipment, etc. Please use the following form when submitting fees.

Each school year, a CYO Consent Form must be completed (signatures required by student and parent) and turned in to the coaching staff or the school office for any student who participates in a CYO sport. The form was also included in the summer mailing packet. Submitting this form once per school year is all that is required. CYO sports include Volleyball, Football, Cheerleading, Basketball, Soccer, Softball, and Track.
Documentation of a current physical must be on file in the school office prior to a student participating in any athletic tryouts, practices, or games. Click this link for the accepted IHSAA physical form. For grade/middle school only pages 1 & 2 need to be completed.
Documentation for emergency contacts must be submitted to the head coach at time of first practice. Please use the attached Student-Athlete Emergency Contact & Medical Care Form.
All persons interested in coaching at St. Charles are asked to complete and turn in a Coaching Letter of Interest form. Click this link to access the form. (SC Coach Nomination Form) Print, complete, and turn in to the school office or to a member of the Athletic Association.
