Student-Athlete Participation Fee Form

The St. Charles Borromeo School Athletic Association annually strives to provide quality programming for our student-athletes, while at the same time remaining financially stable while doing so. To help offset athletic expenses such as uniforms, referees, athletic facilities, equipment, etc., we assess player fees, conduct concession sales, hold annual fundraisers (Kettman Golf Outing, Trivia Night, Pancake Breakfasts, etc.), and solicit private donations.

All Athletic Fee Payments Are Invoiced to Parents’ FACTS Accounts: Parents - Once your student-athlete has been officially added to a team’s roster, athletic fees will be invoiced to, and paid through, your FACTS Tuition and Fees Management Account. Please note that you will receive an email invoice notice at least 10 days before any funds are withdrawn from your account. 

The student-athlete fee is $25 per sport played (Note: $50 for Football) + a one-time Burger King coupon booklet fee.

(Burger King: $30 for the first athlete (10 books); $24 for the second athlete (8 books); and, $18 for the third athlete (6 books) 

We require our student-athletes to participate in the selling of Burger King coupon booklets through the ‘You Can Lend A Hand’ Program, which is sponsored by Quality Dining and endorsed by our Ft. Wayne-South Bend Diocese. Upon participation in their first sport of the year, student-athletes will be charged pre-payment for Burger King booklets, which they will receive near the end of January. Student-athletes can either share the coupons with family and friends, or sell the booklets for $3 each and recoup the earlier assessed fees. Our Athletic Association, through the generosity of the Quality Dining Program, receives $2 of every $3 pre-paid booklet. 

Burger King ‘You Can Lend A Hand’ fees are assessed to the first three student-athletes in a family upon participation in the student athletes’ first sport of the school year. School year 2020-2021 Burger King fees are as follows:

1st Athlete

A one-time $30 fee (10 books)

2nd Athlete

A one-time $24 fee (8 books) 

3rd Athlete

A one-time $18 fee (6 books) 

4th Athlete on up

No booklets charged or received

Concussion and Sudden Cardiac Arrest Training Link & Acknowledgement Form - Online

Required:  This annual training is required by all student-athletes and their parents/guardians before participating in their first sport of the year.  Please review the concussion and sudden cardiac arrest information found here:  Training Link

Required:  After Training is completed by the student-athlete and his parent/guardian, you are required to fill out the following form: Electronic Concussion and Sudden Cardiac Arrest Acknowledgement Form

 

CYO Consent Form - Print, fill out, and return once to the coach of the first sport played

Required:  Each school year, a CYO Consent Form must be completed (signatures required by student and parent) and turned in to the coach of the first sport played each year. Submitting this form once per school year is all that is required.  CYO sports include Volleyball, Football, Cheerleading, Basketball, Soccer, Softball, and Track.

 

Physical - A current physical must be turned each year to participate in athletic activities

Required:  Documentation of a current physical must be on file in the school office prior to a student participating in any athletic tryouts, practices, or games. Click this link for the accepted IHSAA physical form. Take the form to the physician conducting the physical to fill out pages 1 & 2 only and sign. Return the physical form to the school office or to the coach of the first sport played each year. Remember, elementary and middle school students need only have pages 1 & 2 completed by the physician.

COVID-19 Exception For 2020-21. Due to COVID-19 physician-access issues, students with a physical on file from 2019-20 need only to complete the 2020-21 IHSAA Health History Update Questionnaire and Consent & Release Certificate to play.

 

Emergency Contact - Print, fill out, and return to each coach for whom you play

Required:  Documentation for emergency contacts must be submitted to the head coach at time of first practice. Coaches will carry the forms with them to all games. Please use the attached Student-Athlete Emergency Contact & Medical Care Form.

 

St. Charles Coaching and Athletic Board Volunteer Forms

All persons interested in volunteering as either a member of the Saint Charles Athletic Board or as a Coach are asked to complete and turn in a Letter of Interest form.  Click the links below to access the appropriate form. Print, complete, and turn in to the school office (Attn: Athletic Board) or to a member of the Athletic Board.
